"Her condition has deteriorated. She has to be admitted to the hospital," Dr R.C. Golhar of Bombay's G.T. Hospital told Reuters after examining Patkar.
Patkar is the leader of the Narmada Bachao Andolan or Save the Narmada movement group.
The farm land was expropriated by the western India's Maharashtra government to construct the ambitious Sardar Sarovar, a hydroelectric dam.
Six others have joined her in the fast which she began on September 17. Since then, they have been drinking only water.
Sardar Sarovar is the biggest of around 3,200 dams spread across three states to be built on the 1,300 km (800 mile) Narmada river.
Resting on a bed in a tent, Patkar appeared frail but told Reuters that she felt normal.
She has fasted before in support of environmental causes - once for 22 days.
